Abstract
CASE REPORT PADA AKSEPTOR KONTRASEPSI SUNTIK DEPO MEDROXY PROGESTERONE ACETATE (DMPA) DENGAN EFEK SAMPING KENAIKAN BERAT BADAN DI WILAYAH KERJA PUSKESMAS KAMAL KABUPATEN BANGKALAN Kenaikan berat badan menjadi salah satu efek samping penggunaan kontrasepsi suntik DMPA yang berpotensi menimbulkan rasa tidak nyaman secara fisik, mengganggu aktivitas harian, serta memengaruhi kondisi psikologis akseptor. Banyak akseptor belum mengetahui penanganan yang tepat terhadap penurunan berat badan, sehingga diperlukan asuhan kebidanan untuk membantu mengatasi masalah tersebut. Riset ini dimaksudkan untuk menguraikan manajemen pelayanan kebidanan pada pengguna kontrasepsi suntik DMPA yang mengalami efek samping berupa peningkatan berat badan. Mengacu pada catatan Puskesmas Kamal Januari 2025, diketahui 7 dari 12 penerima layanan mengalami pertambahan bobot tubuh. Pendekatan yang digunakan adalah metode deskriptif melalui studi kasus. Pelayanan kebidanan diberikan kepada Ny. S, 34 tahun, yang mengalami peningkatan bobot tubuh lebih dari 10 kg selama ±1,5 tahun penggunaan kontrasepsi suntik DMPA. Data diperoleh melalui teknik wawancara, pengamatan, serta pemeriksaan secara langsung. Tindakan dilakukan dalam tiga kali kunjungan selama satu bulan dengan metode pengkajian, intervensi, evaluasi, dan pencatatan. Tindakan yang diberikan meliputi edukasi mengenai efek samping kontrasepsi, pemberian menu diet sehat sesuai kebutuhan gizi ibu, anjuran jalan pagi selama 20– 30 menit, serta konseling terkait pemilihan metode kontrasepsi lain. Evaluasi menunjukkan adanya penurunan berat badan dan akseptor menyatakan keinginan untuk beralih ke metode kontrasepsi MOW. Kesimpulan dari studi ini adalah bahwa asuhan kebidanan yang tepat dan edukatif dapat membantu mengatasi efek samping kenaikan berat badan serta meningkatkan pemahaman akseptor terhadap pemilihan metode kontrasepsi yang sesuai. Kata Kunci : Kontrasepsi suntik DMPA, Kenaikan berat badan CASE REPORT ON A CONTRACEPTIVE INJECTABLE DEPO MEDROXYPROGESTERONE ACETATE (DMPA) ACCEPTOR WITH THE SIDE EFFECT OF WEIGHT GAIN IN THE WORKING AREA OF KAMAL COMMUNITY HEALTH CENTER, BANGKALAN REGENCY Weight gain is one of the side effects of using the DMPA injectable contraceptive that potentially causes physical discomfort, interferes with daily activities, and affects the psychological condition of the acceptor. Many acceptors are unaware of the proper management for weight loss, thus requiring midwifery care to help overcome this issue. This research is intended to outline the management of midwifery services for users of the DMPA injectable contraceptive who experience weight gain as a side effect. Referring to the records of the Kamal Community Health Center in January 2025, it was found that 7 out of 12 service recipients experienced weight gain. The approach used is the descriptive method through a case study. Midwifery care was provided to Mrs. S, 34 years old, who experienced a weight increase of more than 10 kg during approximately 1.5 years of using the DMPA injectable contraceptive. Data was obtained through interview techniques, observation, and direct examination. The actions were carried out in three visits over one month using the methods of assessment, intervention, evaluation, and documentation. The actions given included education about contraceptive side effects, provision of a healthy diet plan according to the mother's nutritional needs, recommendation for morning walks for 20–30 minutes, and counseling related to the selection of other contraceptive methods. Evaluation showed a decrease in weight and the acceptor expressed the desire to switch to the MOW contraceptive method. The conclusion of this study is that appropriate and educational midwifery care can help overcome the side effect of weight gain and enhance the acceptor's understanding in choosing a suitable contraceptive method.
