Abstract
Too close a pregnancy spacing, a history of Intra Uterine Fetal Death (IUFD), accompanied by anemia and Chronic Energy Deficiency (CED) are risk factors that need to be addressed appropriately. This study aims to describe midwifery care for multigravida mothers in the second trimester with very high risk at PMB “N” Geger Bangkalan. The study used a descriptive method with a case report approach on one multigravida pregnant woman in the second trimester with a very high risk pregnancy. Data were obtained through interviews, observations, physical examinations, supporting examinations and documented using SOAP. The implementation of care took into account ethical principles such as informed consent, beneficence, non-maleficence, anonymity, and confidentiality. Pregnant women with gestational age of 23–24 weeks had a history of IUFD and a pregnancy spacing of less than two years. The examination showed normal vital signs, hemoglobin level of 10.9 g/dL, and MUAC 22.5 cm indicating CED. Fetal heart rate and uterine fundal height were appropriate for gestational age. The diagnosis was established as G2P1A0 gestational age 23–24 weeks, a single fetus living intrauterine with mild anemia and CED. Care included home visits, monitoring the condition of the mother and fetus, compliance with consumption of Fe tablets, folic acid, Multiple Micronutrient Supplement (MMS), and Supplemental Feeding (PMT) in the form of risol, dragon fruit, and full cream milk. After the intervention, hemoglobin levels and body weight increased, while the LILA had not reached the normal limit. During the monitoring, no signs of pregnancy danger were found and the condition of the mother and fetus remained good. Continuous midwifery care helps support the improvement of anemia conditions and monitoring the well-being of the mother and fetus, while the management of CED requires time and continuous intervention. Intensive antenatal monitoring, nutrition education, supplementation compliance, and collaboration with nutritionists need to be improved in very high-risk pregnant women to prevent maternal and perinatal complications.
