Abstract
CASE REPORT PADA MULTIGRAVIDA TRIMESTER III DENGAN SIMFISIS PUBIS DISFUNGSI DI PMB BIDAN “D” KLAMPIS KABUPATEN BANGKALAN xvi + 94 Halaman + 3 Tabel + 8 Lampiran Simfisis Pubis Disfungsi (SPD) merupakan kondisi muskuloskeletal pada kehamilan yang ditandai dengan rasa nyeri di area simfisis pubis. Kondisi ini berpotensi membatasi gerakan dan mengganggu aktivitas harian ibu. Laporan Tugas Akhir ini berfokus pada penggalian dan penguraian penatalaksanaan kebidanan pada ibu hamil trimester III dengan kondisi SPD menggunakan kerangka manajemen kebidanan SOAP. Studi ini mengadopsi metode deskriptif melewati pendekatan laporan kasus tunggal pada seorang ibu multigravida di trimester III yang menderita SPD. Kasus ini diambil di PMB Bidan “D” Klampis, Kabupaten Bangkalan, antara Juni hingga Juli 2026. Subjek penelitian adalah Ny. J, seorang ibu dengan kehamilan kedua (G2P1A0) berusia 37–39 minggu. Pengumpulan data dilakukan selama empat kali kunjungan melalui wawancara (anamnesis), observasi, pemeriksaan fisik dan obstetri serta dokumentasi. Asuhan kebidanan didokumentasikan dengan mengacu pada data subjektif, objektif, analisis, dan rencana penatalaksanaan. Pengkajian awal pada kunjungan pertama mengidentifikasi Ny. J merasakan nyeri di bagian depan panggul yang menjalar ke selangkangan. Nyeri ini terasa terutama saat berjalan, naik tangga, menopang berat badan pada satu kaki, dan saat mengubah posisi tidur. Nyeri ini menimbulkan ketidaknyamanan, sedikit hambatan aktivitas, serta kecemasan terkait proses persalinan. Pemeriksaan obstetri mengonfirmasi kondisi ibu dan janin tetap stabil, dengan janin dalam posisi kepala dan kepala janin telah turun memasuki panggul. Intervensi dijalankan secara progresif sesuai hasil evaluasi dan perubahan kondisi ibu, mencakup pemberian edukasi mengenai SPD, penyesuaian aktivitas, istirahat yang memadai, latihan prenatal yoga, penggunaan penyangga kehamilan (pregnancy support belt), terapi kompres hangat selama 15–20 menit, pemantauan intensitas nyeri serta kondisi ibu dan janin, dan pemberian edukasi persiapan persalinan serta tanda-tanda bahaya. Berdasarkan rangkaian asuhan selama empat kunjungan, disimpulkan bahwa manajemen kebidanan dengan pendekatan SOAP terhadap Ny. J yang mendapati SPD berhasil menurunkan intensitas nyeri dari skala 4 menjadi 2, menaikkan kenyamanan dan kemampuan gerak, serta menjaga kesehatan ibu dan janin. This condition can potentially limit movement and disrupt the mother's daily activities. This Final Project Report aims to explore and detail the midwifery management of a thirdtrimester pregnant woman experiencing SPD, utilizing the SOAP midwifery management framework. This study adopts a descriptive method through a single case report approach on a multigravida mother in her third trimester suffering from SPD. The case was managed at Midwifery Clinic "D" Klampis, Bangkalan Regency, between June and July 2026. The study subject was Mrs. J, a mother in her second pregnancy (G2P1A0) at 37–39 weeks gestation. Data were collected through interviews (anamnesis), observation, physical and obstetric examinations, and documentation during four follow-up visits. Midwifery care was documented by referring to subjective, objective, analysis, and management plan data. The initial assessment during the first visit identified that Mrs. J experienced pain in the front of her pelvis radiating to the groin. This pain was particularly noticeable when walking, climbing stairs, bearing weight on one leg, and when changing sleeping positions. The pain caused discomfort, mild activity limitations, and anxiety regarding the labor process. Obstetric examinations confirmed that the condition of the mother and fetus remained stable, with the fetus in a cephalic presentation and the fetal head engaged in the pelvis. Interventions were implemented progressively based on evaluation results and changes in the mother's condition, including education about SPD, activity modification, adequate rest, prenatal yoga exercises, the use of a pregnancy support belt, warm compress therapy for 15–20 minutes, monitoring of pain intensity and the condition of the mother and fetus, and education on labor preparation and danger signs. Based on the series of care provided over four visits, it was concluded that midwifery management using the SOAP approach for Mrs. J experiencing SPD successfully reduced pain intensity from a scale of 4 to 2, improved comfort and mobility, and maintained the health of the mother and fetus.
