Abstract
Kurang Energi Kronis (KEK) pada ibu hamil tetap menjadi isu kesehatan yang umum di Indonesia. Kondisi ini berpotensi menimbulkan konsekuensi serius terhadap kesehatan ibu dan janin, termasuk peningkatan risiko kelahiran bayi dengan berat badan lahir rendah (BBLR), persalinan prematur, serta mortalitas ibu dan bayi. Menurut data dari Survei Kesehatan Indonesia (SKI) tahun 2023, angka prevalensi KEK pada ibu hamil di Indonesia mencapai 17,3%. Di area pelayanan Puskesmas Bangkalan, kasus KEK terus teridentifikasi secara rutin setiap tahun, sehingga memerlukan intervensi kebidanan yang berkesinambungan. Tujuan dari kajian ini untuk menggambarkan pelayanan kebidanan yang diberikan kepada ibu hamil primigravida pada trimester kedua yang mengalami KEK di Puskesmas Kamal. Metodologi yang diterapkan adalah deskriptif dengan pendekatan case report, yang dilakukan di wilayah kerja Puskesmas Kamal, Kabupaten Bangkalan. Subjek penelitian meliputi satu individu ibu hamil yang memiliki lingkar lengan atas (LILA) kurang dari 23,5 cm dan Indeks Massa Tubuh (IMT) di bawah 18,5 kg/m², yang telah didiagnosis dengan KEK. Data yang dikumpulkan mencakup informasi subjektif, data objektif, hasil analisis, serta rencana dan tindakan pelayanan kebidanan yang disesuaikan dengan kondisi pasien berdasarkan persetujuan informed consent. Temuan dari peneliian ini mengindikasikan adanya perbaikan status gizi ibu melalui tiga kali kunjungan: kenaikan berat badan dari 37 kg menjadi 40 kg, peningkatan IMT dari 16,4 kg/m² menjadi 17,7 kg/m², serta kenaikan taksiran berat janin (TBJ) dari 1.240 gram menjadi 1.550 gram. Intervensi meliputi edukasi gizi seimbang, konsumsi tablet Fe dan MMS, serta penyuluhan makanan bergizi lokal secara konsisten. Dapat disimpulkan bahwa intervensi kebidanan yang mencakup pemantauan rutin, pemberian makanan tambahan (PMT), suplementasi Multiple Micronutrient Supplement (MMS), serta penerapan pendekatan holistik dengan melibatkan tenaga kesehatan, dukungan keluarga, dan peran aktif ibu, terbukti berkontribusi dalam meningkatkan kondisi status nutrisi ibu hamil dengan Kurang Energi Kronis (KEK) serta mendukung tercapainya kehamilan yang sehat. Kata kunci : Kurang Energi Kronik: Trimester II ABSTRACT Chronic Energy Deficiency (CED) in pregnant women constitutes a critical public health issue in Indonesia, especially in areas where access to sufficient nutrition and healthcare is limited. This condition introduces severe risks to both maternal and fetal health, such as an elevated incidence of low birth weight (LBW), preterm birth, and higher rates of maternal and neonatal mortality. Data from the 2023 Indonesian Health Survey (SKI) indicate that the prevalence of CED among pregnant women in Indonesia stands at 17.3%. Furthermore, in the Bangkalan Public Health Center region, cases of CED are persistently identified each year, highlighting the essential need for sustained midwifery care and interventions This study seeks to delineate the midwifery care administered to a primigravida in her second trimester, who has been diagnosed with Chronic Energy Deficiency (CED), within the operational jurisdiction of the Kamal Health Center in Bangkalan Regency. It utilized a descriptive methodology incorporating a case study approach, implemented in the service area of the Kamal Health Center. The subject comprised a single primigravida in the second trimester afflicted with CED, defined by a mid-upper arm circumference (MUAC) below 23.5 cm and a Body Mass Index (BMI) under 18.5 kg/m². Data gathering involved subjective and objective assessments, analytical evaluations, and tailored midwifery interventions, all undertaken with the participant's informed consent. The outcomes indicated enhancements in the mother's nutritional profile following three sessions: body weight increased from 37 kg to 40 kg, BMI advanced from 16.4 kg/m² to 17.7 kg/m², and estimated fetal weight (EFW) progressed from 1,240 grams to 1,550 grams. The implemented strategies encompassed educational sessions on balanced nutrition, routine administration of iron tablets and multiple micronutrient supplements (MMS), and ongoing advice regarding accessible local nutritious foods. In conclusion, midwifery interventions such as regular monitoring, provision of supplementary foods (PMT), MMS supplementation, and a holistic approach involving healthcare workers, family support, and active maternal participation contributed positively to improving nutritional status and promoting a healthy pregnancy for women with CED.
