Abstract
Kontrasepsi suntik DMPA mengandung 150 mg hormon progesteron dan bekerja selama 12 minggu untuk mencegah ovulasi. Spotting terjadi akibat ketidakseimbangan hormon dalam tubuh dan umumnya muncul pada pengguna kontrasepsi suntik DMPA setelah 3–4 siklus penyuntikan. Hal ini menyebabkan pelebaran pembuluh darah kecil pada endometrium, sehingga pembuluh darah menjadi lebih rapuh dan mengakibatkan perdarahan bercak. Tujuan laporan kasus ini adalah untuk memberikan asuhan kebidanan pada seorang pengguna jangka panjang kontrasepsi suntik DMPA yang mengalami spotting, yang dilakukan di Puskesmas Kamal, Kabupaten Bangkalan. Asuhan dilakukan dengan pendekatan laporan kasus dari Januari hingga Juni 2025. Data dikumpulkan melalui wawancara, pemeriksaan fisik, dan observasi kartu K4, dengan persetujuan (informed consent) dari partisipan. Asuhan kebidanan diberikan kepada Ny. I, seorang pengguna jangka panjang kontrasepsi suntik DMPA yang mengalami spotting, melalui tiga kali kunjungan rumah. Pada kunjungan pertama, ditemukan spotting dan kemerahan pada vagina. Penatalaksanaan awal meliputi edukasi mengenai spotting akibat DMPA, kebersihan vulva, serta pemberian kontrasepsi oral kombinasi selama satu siklus. Pada kunjungan kedua, spotting telah berhenti, dan pasien dianjurkan untuk menyelesaikan siklus pil. Pada kunjungan ketiga, pasien melaporkan tidak mengalami spotting lagi dan tetap melanjutkan penggunaan DMPA tanpa komplikasi. Asuhan meliputi konseling, edukasi kebersihan vulva, serta pemberian kontrasepsi oral kombinasi selama satu siklus. Spotting berhenti pada hari ketiga terapi, menunjukkan bahwa intervensi efektif, dan pasien melanjutkan penggunaan DMPA tanpa keluhan lebih lanjut. This leads to dilation of small blood vessels in the endometrium, making the veins more fragile and resulting in spotting.The purpose of this case report is to provide midwifery care to a long-term DMPA injectable contraceptive user experiencing spotting, conducted at Kamal Public Health Center, Bangkalan Regency.Method: The care was conducted using a case report approach from January to June 2025. Data were collected through interviews, physical examinations, and K4 card observations, with informed consent obtained from the participant.Result: Midwifery care was provided to Mrs. I, a long-term user of DMPA injectable contraception who experienced spotting, through three home visits. On the first visit, spotting and vaginal redness were observed. Initial management included education on DMPA-related spotting, vulva hygiene, and a one-cycle course of combined oral contraceptives. By the second visit, spotting had stopped, and the patient was advised to complete the pill cycle. On the third visit, the patient reported no further spotting and continued DMPA use without complications.Conclusion : The care included counseling, vulva hygiene education, and one cycle of combined oral contraceptive pills. Spotting stopped by the third day of therapy, indicating the intervention was effective.
